Q. Am I a registered patient with you?
A. Patients no longer 'register' with a dentist. Unlike your GP, it is no longer possible to remain a permanent NHS patient at a dental practice unless you regularly attend appointments and check ups. NHS places are limited, therefore, if you do not attend for more than 18 months, you will no longer be a patient of the practice and will have to re-join. These are the NHS rules for all NHS dental practices, as set by the government in 2005 when the way dentists were paid by the NHS changed. To become a patient at Revive Dental Care, you must follow our simple four step joining plan and regularly attend your check-up appointments. Q. I am pregnant, do I have to pay for dental treatment?
A. If you are pregnant, or you have had a baby in the last 12 months, you are entitled to free NHS dental treatment providing that you start a course of treatment while pregnant, or during the 12 months after giving birth. To claim your free dental treatment you will need to show your dentist a valid maternity exemption certificate, or card, issued by the Prescription Pricing Authority. To apply for your maternity exemption card, ask your doctor, midwife, or health visitor for Form FW8. Your doctor, midwife, or health visitor will sign the form to confirm your statement.
