NHS Patients
We are currently accepting new NHS patients at Revive Dental Care Centres. If you would like to find an NHS dentist for your family, simply complete the four steps below to register with an NHS dentist in your area. Alternatively you can visit our Monton NHS dental clinic or telephone our reception on 0161 789 1557.
Step 1 Complete a Medical Questionnaire Form: one form per patient please. Click the links below to download the appropriate medical questionaire forms:

Step 2 Email the completed form to reception.monton@revivedentalcare.co.uk
Alternatively, you can print out and deliver your completed form (in person or by post) to your local dental practice.
Step 3 Call the practice to book your Joining Examination Appointment.
Step 4 Attend your Joining Examination Appointment with your dentist. You should also complete a personal Smile Assesment before your first appointment.
